How we use this information

We use these details to identify the pupil, verify your authority, process the request, contact you and keep necessary audit records. Completed requests are restricted to authorised staff. We may share relevant information with the school and service providers needed to complete the request. We do not use this form information for marketing and retain only what is necessary under our data protection and legal obligations.
